• Have you previously had an MRI?

• Have you EVER had metal in either or your eyes?

• Do you have a pacemaker or pacemaker wires?

• Have you ever had heart or brain surgery?

• Have you had aneurysm surgery?

• Do you have any surgical hardware in your body such as clips, wires, etc.?

• Have you had surgery in the past six weeks?

• Do you have any type of implant such as eye, ear, breast, penile, etc.?

• Do you have any type of metal in your body such as bullets, shrapnel, etc.?

• Do you have a neuro-stimulator in your body?

• Do you have renal failure or renal insufficiency?

• Are you nursing or pregnant?
